Three ways to say it
“hi, i'm looking at my statement and there's a charge from [merchant] for $[amount] on [date] that i don't recognize at all, i think this might be fraud”
Unrecognized charge from [merchant] on [date] for $[amount]. Requesting dispute and card reissue.
“hi! so i noticed i was charged for the same thing twice last week, could you please refund the duplicate charge?”
Duplicate charge from [merchant] on [date] and [date] for $[amount] each. Please refund one.
“hi, i paid [merchant] on [date] for [service] but they never delivered, i've tried to reach them and they won't respond, i'd like to dispute this”
Paid [merchant] on [date] for $[amount]. Service not delivered. Merchant unresponsive. Dispute please.
